- What is this notice about? The IRS is addressing, or needs more information about, your business, estate, or trust entity. This is needed to verify information reported on your Form SS-4, Application for Employer Identification Number (EIN).
- What should you do if you receive this notice? If you're affiliated with the entity, respond within 30 days with all requested information. For estates, include a death certificate. For trusts, provide a Certificate of Trust or relevant trust documents. If you're not affiliated, indicate so on the letter, provide your name, and return it within 30 days.

- What will happen after I provide my response? The IRS will contact you within 180 days if they have questions. If not, no action is required. It will take about 9 weeks to make a determination and process any returns once they've reviewed your response.
